About Leanne 

Leanne is an enthusiastic and motivated individual who has a strong pharmaceutical background (non-clinical), with experience in minor ailments and patient signposting. 

Aims of the role

To support our member practices increase the uptake of patient signposting to alternative services such as District Nurses, Pharmacy minor ailments, First Contact Practitioners and various other services ensuring patients get the Right Care, First Time.

Leanne is currently training in Screening including Bowel Screening, Breast Screening and Cervical Screening. She aims to support our member practices with uptake of screening, and to support patients with the screening process.

Leanne also currently supports patients who are registered with their practice as a ‘Carer’. In 2025, Leanne will be offering carers assessments to any patients registered as carer, to ensure unpaid carers are receiving the help and support they need.

The process and what to expect as a patient

As you may be aware Warrington Innovation Network promotes the ‘Right Care, First Time campaign’ this ensures that patients are triaged and given the ‘Right Care, First Time’ without necessarily needing to see a GP every time. If you are triaged by Leanne, you may be signposted to alternative services.

As Leanne is working on Screening and Carers Assessments, you may receive an SMS message with further information, or to confirm if you are still registered as an unpaid carer. It is important you respond to these messages so your medical records are updated.
