COVID 19 Vaccinations
Integration of additional roles into the PCN eg: Clinical Pharmacists, Physiotherapists, Paramedics, Social Prescribers, Mental Health Practitioner, Care Coordinators and regular care home visits from clinical team.
Alignment of Care Homes with specific Practices to ensure continuity of care for patients/residents.
