What is a Primary Care Network (PCN)?
A PCN is a group of GP practices working together to deliver integrated services to their local population. The GP practices that work together within Warrington Innovation Network include 4Seasons Medical Centre, Chapelford Medical Centre, Culcheth Medical Centre, Guardian Medical Centre, Parkview Medical Practice, Springfields Medical Centre and Westbrook Medical Centre.
What is the purpose of the GP patient survey?
- To monitor our patient's ease of access to their GP Practice
- Guage our patient's satisfaction with the appointments they have been offered
- To monitor our patient's awareness of clinicians that work within our practices
What do you hope to achieve in sending this survey?
- Make improvements in our patient's satisfaction with making appointments
- Increase awareness of other clinicians that work within our practices that are experts in their fields
- Hear patient's views and opinions
Who will see my response to the survey?
- The survey is anonymous, and only staff working at Warrington Innovation Network will see the patient's individual responses. However, we aim to share the survey results and how we hope to make improvements with patients via the PCN newsletter.
Do I need to complete the survey?
- No, you do not need to complete the survey. It is optional.
Will it cost me anything to complete the survey?
- No, it is free to complete the survey
Is this the only survey you will ask me to complete?
- We will follow up the survey with another GP patient survey within the next year to see if patient's opinions have changed. You may also receive a friends and family test when visiting your practice.
